This position is hybrid/remote



The Department of Medicine, Division of Endocrinology, Diabetes and Metabolism has an exciting opportunity for Two(2) Nurse Navigators to work on the UHealth Campus. The Nurse Navigator is the initial point of contact for a patient entering the health system. The Nurse Navigator assures timely scheduling of the first appointment, coordination of care after completion of the first appointment, and will support the patient throughout the care continuum. The role also serves as a liaison between patients, families, caregivers, and the multidisciplinary care team. Furthermore, the Nurse Navigator demonstrates commitment to quality patient care, implements creative and innovative ways to meet the diverse needs of the patients and ensures best practices.



Triages sick patient calls to the appropriate provider(s) and assures timely responses within 48 hrs.



Retrieve clinical related messages and return calls within 48 hours.



Manage and resolve UChart messages.



Submit lab order requests. Interprets laboratory test results of patients.



Process prescription refills. Process authorizations for medications and procedures. Process appeals and peer to peer reviews.



Assess barriers to care and refers to support services.



Review outside medical records for appropriate scheduling.



Use clinical protocols to refer patients to the appropriate specialist for diagnosis or treatment.



Provide emotional support, and counseling, related to the clinical situation.



Responsible for developing and enhancing processes with the aim to improve the clinical experience for referred patients or physicians.



Help patients and their families in obtaining referrals to a specific specialist.



Take care of all patient requests.



Serve as the liaison between patients and providers.



Maintains professional knowledge by affiliating with professional and technical organizations, and participating in applicable continuing education programs, conferences, seminars, and workshops.

MINIMUM QUALIFICATIONS



Bachelor?s degree in Nursing



Two to three years of nursing work experience, as a health or patient educator, or a pharmaceutical representative.



Valid Florida RN License



Current American Heart Association BLS and ACLS
