Position Summary

Our Licensed Clinical Social Worker position represents an innovative step forward in the relationship between primary care and behavioral health in our community. The LCSW is responsible for directing and shaping the integrated behavioral health services of the clinic in collaboration with the primary care team and as an expert in Behavioral Health, will work closely with the primary care providers to understand the root cause of patient's psychosocial issues and implement effective treatment plans.

The LCSW supports the enhancement and expansion of our behavioral health programs by collaborating on patient care as a part of an interdisciplinary care team-based approach with physicians, other providers, nurses, and health care professionals with a goal of improving health care quality. Together with physician leadership, the LCSW will be accountable for maintaining quality integrated behavioral health programs for patients and staff.

As the Licensed Clinical Social Worker, you will have the opportunity:

  • Perform individual patient assessment of needs and develop patient-specific care plans, collaboratively with the primary care team to ensure high-quality and well-rounded patient care.
  • Collaborate with providers and the care team to develop and implement structured, evidence-based behavioral health treatment as part of a coordinated treatment plan with a goal of improving quality outcomes.
  • Educate patients on resource available to them and conduct follow-up evaluations.
Qualifications

Education
  • Master's degree in Social Work required
Required Experience
  • Experience with psychosocial assessment, individual therapy, evidence-based counseling, and crisis intervention
  • Experience working in the medical social work or community mental health field and knowledge of Integrated Behavioral Health in the community healthcare system
  • Familiarity and comfort working with health-related issues, substance abuse, and history of trauma
Licensure/Certifications
  • LCSW: Licensed Clinical Social Worker by the State of Oregon required upon start
  • BLS: Basic Life Support for the Healthcare Provider certified through the American Heart Association, preferred
  • Certification through the National Association of Certified Social Workers and/or the Academy of Certified Social Workers, preferred
  • Crisis Intervention Training and/or certification, preferred
