Public Health Nursing is provided throughout Becker County and is focused on individuals,

families, and their communities. It devises systems change to positively impact the health of all.

Public Health Nursing is population based, focusing on the entire population. Public Health

Nurses assess and analyze the broad determinants of health, and services are prevention focused.

Nurses collaborate, assess needs, and provide independent nursing interventions in

the community which may include schools, homes, clinics, shelters, nursing homes, and other

community settings.

Knowledge, Skills, and Abilities:

Thorough knowledge of the application of nursing practices and considerable knowledge of public health nursing theory. Ability to apply principles of public health to communities, families, and individuals; to provide skilled nursing care and assessment as necessary. Ability to establish and maintain effective relationships with clients, agency staff members, other agency professional representatives, and the general public; to apply previously acquired knowledge and skills to new situations; to communicate effectively, both orally and in writing; to make independent decisions; and to utilize community resources. Knowledge of agency programs, policies and procedures. Knowledge of office practices, procedures, and equipment. Knowledge of software programs as they pertain to this position.

Duties:

Primary duty to be assigned:

The SNBC (Special Needs Basic Care) Coordinator ensures access, provides advocacy, and helps each member navigate complex healthcare delivery systems. The SNBC coordinator works in partnership with their clients to develop individualized comprehensive plan of care that will assist each client in addressing medical, social, behavioral, educational, and other service needs.

Other duties as assigned:

-Provide comprehensive nursing assessments in assigned program. Information and referral to health and human services programs through home visits and in community settings to adults and families to promote positive health outcomes.

-Provide ongoing case management for clients in assigned program.

-Maintain case notes and client files to meet State and Federal requirements.

-Health teaching for positive health outcomes and to prevent illness. Investigation of disease and other health events.

-Promote, develop, and maintain good working relationships with the public, the community, and other agencies. Respond promptly and accurately to public requests and inquiries.

- Respond to disasters and assist communities in recovery to prevent the spread of disease or injury by recognizing deviations from the norm that might indicate an emergency.

Qualifications:

Registered Nurse Licensure in MN required; BSN/PHN Certification preferred, but not required. A minimum of 1-2 years of Public Health experience preferred. Valid driver?s license required.

Compensation and Benefits:
The compensation range for this position is $67,324 to $88,058 annually, depending on experience. This position is eligible for a wide range of competitive benefits, including state pension plan, vacation time, sick time, personal and holiday leave time. In addition, the position is eligible for the County's cafeteria health plan, consisting of three health plans and carrier options; Dental, Vision, and Life insurance; Health Savings and Flexible Spending accounts; Long-term and Short-Term Disability; Supplemental Accident, Hospital, and Critical Illness plans; and Public Service Loan Forgiveness (PSLF) program. The County contributes monthly health benefits to aide in the rising of insurance costs with the contribution rate evaluated annually. Additionally, employees have the option to take part of this contribution as cash in lieu of benefits. This position is also eligible for part-time telecommuting after a 6-month probationary period.
