Job Overview

We are currently seeking a committed and proactive Registered Nurse (RN) to become a valued member of our team in a hybrid capacity, combining remote work with essential in-person visits. Key Responsibilities
? Manage patient care and attend necessary medical appointments.
? Coordinate hospital visits, procure medical equipment, arrange home modifications, and facilitate additional supportive services.
? Operate independently, maintaining a flexible schedule while traveling locally as required.
? Deliver outstanding care and support to patients, ensuring optimal treatment outcomes.
? Engage in travel for in-person visits, which will comprise 50%-75% of your time.

Required Skills
? Active Oregon RN License.
? Valid Oregon Driver's License along with Auto Insurance.
? Proficiency in Microsoft Word and Excel.
? Bilingual proficiency in Spanish is advantageous.
? Strong ability to work autonomously while also functioning as part of a collaborative team.

Qualifications
? A minimum of 1 year of Nursing Experience (preferred).
? A minimum of 1 year of Case Management Experience (preferred).
? Active RN License is required.
? Openness to travel locally for in-person visits (50%-75%).
