Occupational Health LVN Case Manager -Remote

We are seeking a dedicated Nurse Case Manager to join our team. The ideal candidate will have substantial experience in case management and will work closely with our company's safety teams, onsite medics, paramedics, nurses, and nurse practitioners to ensure the highest standard of care for our employees. This role is crucial in reducing the rate of recordable injuries and ensuring accurate reporting to the appropriate agencies.
DUTIES AND RESPONSIBILIITES:

? Ensure appropriate and costeffective healthcare services leading to a medically rehabilitated individual who is ready to return to an optimal level of work and functioning.
? Using clinical/nursing skills to help coordinate the individual?s treatment program while maximizing cost containment.
? Serving as an intermediary to interpret and educate the individual on his/her disability, and the treatment plan established by the case manager, physicians, and therapists. Facilitate treatment between providers and worker.
? Ensure proper reporting of injuries to relevant agencies and maintain accurate records.
? Work closely with onsite medics, paramedics, nurses, and nurse practitioners to guide cases, ensuring appropriate treatment and care for all employees.
? Perform triage and refer employees to appropriate medical care based on their needs.
? Resource for claims personnel when needing providers in certain locales.
? Partner with employers determining modified work.
? Provide accurate and quality documentation meeting Best Practices timeframes.
? Reporting billing hours in accordance with case activity and billing practices. Following specific account instructions.
? Demonstrates a level of professionalism within the workplace and in dealing with injured workers reflects positively on the company as a whole.

? Requires excellent time management and organizational skills.
? Spanish speaking a plus
? Must have reliable transportation
? Knowledge of basic computer skills including Excel, Word, and Outlook Email
? Must have local unencumbered driver?s license and proof of automobile insurance.
Effective written and verbal communications skills
EDUCATION AND EXPERIENCE QUALIFICATIONS: ? Associates degree in qualified nursing program, required; Bachelor?s in Nursing, preferred ? Current, unencumbered Licensed Vocation Nurse license in the resident state; CCM, COHN preferred. ? 3 year clinical experience in a hospital setting, required; Worker?s compensation or disability management experience is preferred ? Previous field case management experience, preferred
Certification in Case Management (CCM) or Occupational Health Nursing (COHN).
? Experience working in a remote or fieldbased setting.
? Knowledge of current trends and practices in occupational health and safety
Work location: Remote
Expected Hours: 40 hours per week
On Call: 1 weekend a month
