As part of an interdisciplinary team, the nurse practitioner will engage in health risk assessments, transitions of care, chronic disease management, and palliative care support, ensuring that patients receive the right level of care at the right time. In this role, the nurse practitioner will conduct in-home longitudinal care for medically complex patients, aiming to improve clinical outcomes and decrease healthcare utilization. Responsibilities
? Perform home visits with patients enrolled in chronic care risk program, conducting comprehensive evaluations and targeting risk factors for disease exacerbations and hospitalizations.

? Coordinate with patients' other providers, including PCPs, specialists, and health plan case managers, to ensure collaborative care towards achieving health goals.

? Share after-hour on-call coverage with other providers on the team.

? Work closely with field and office RNs and medical assistants to ensure quality care delivery.
,
? Document acute and chronic healthcare issues through patient encounters in the EMR system, ensuring thorough and timely documentation.
,
? Educate patients and/or their families on chronic medical conditions, preventative care, and medication adherence compliance.
,
? Participate in weekly telephonic clinical meetings and occasional web-based compliance trainings, with some on-site training required.
,
? Prescribe medications, order labs or diagnostic testing, and make subspecialty referrals in collaboration with the community PCP.
,
? Review patient records for past and present health issues, updating charts accordingly.
,
? Collaborate with the supervising physician and other healthcare team members to ensure outstanding patient care is delivered.
,
? Interpret laboratory data and diagnostic testing to assist in the diagnosis of medical conditions.
,
? Screen for ADL/IADL deficits, fall risk, depression, cognitive impairment, and nutritional health during assessments.

? Conduct medication reconciliation at every visit.
,
? Perform other duties as assigned by the supervising physician and/or manager.

Requirements
? Graduated from an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Nurse Practitioner.

? Active, unrestricted NP license in the respective state.
,
? Board certified by the ANCC or AANP in geriatrics, with current RN and NP licensure in the state of practice.

? Current BLS certification; ACLS preferred.

? Ability to be credentialed by Medicare, Medicaid, and other private insurance companies.

? DEA license and State Controlled Substance License upon hire or within 6 months of hire.

? Experience with electronic medical records and proficiency with Microsoft Office applications.

? Prior clinical experience in geriatric, adult, or family medicine is preferred.

? Strong background/interest in primary care and preventative medicine.

? Valid state driver's license.

? Highly motivated, self-directed professional with strong organizational skills and comfortable working independently.

? Excellent clinical assessment and analytical skills.

? Effective leadership and interpersonal skills, with the ability to facilitate problem resolution.

? Excellent verbal and written communication skills.
