Healthcare in Action is dedicated to serving patients experiencing homelessness through an innovative "street medicine" approach. This involves utilizing a medically equipped van to deliver essential medical care directly to individuals in need, often in locations such as parking lots, encampments, and parks. The LVN Clinical Support Partner plays a crucial role in this mission by supporting clinicians in monitoring patients who are at risk of homelessness and assisting them in managing their health. This position requires performing basic clinical care tasks, including obtaining vital signs, drawing blood, and providing basic wound care at the San Mateo Navigation Center in Redwood City, as well as in our mobile unit in the surrounding areas. In this hybrid role, the LVN will coordinate care for patients by arranging ride-share services, picking up prescriptions, and scheduling appointments. Responsibilities also include assisting with inventory management, ordering supplies, and triaging patient phone calls and new consults. The LVN will conduct physical exams and assessments, administer vaccinations, and draw blood for lab tests when necessary. They will also provide wound care, monitor patients, and assist in developing individualized care plans in collaboration with patients, peer navigators, therapists, and primary care physicians. The LVN will educate patients on medical issues, consult with healthcare providers as needed, and ensure that primary care physicians are informed about patients' health conditions and progress. This role emphasizes the importance of creating a trusting environment for patients, maintaining a safe and efficient workspace, and adhering to company policies regarding lab specimens and biohazardous waste. The LVN will document patient interactions accurately and participate in quality assurance and improvement programs, contributing to the overall goals of Healthcare in Action.
Responsibilities
? Conduct physical exams and assessments including obtaining EKGs, administering vaccinations, and drawing blood for lab tests as necessary.

? Provide wound care and patient monitoring.

? Assist with organizing and completing individualized Care Plans for each patient.

? Develop care plans in collaboration with patients, peer navigators, therapists, and primary care physicians.

? Educate and advise patients on medical issues as needed.

? Consult with physicians, physician assistants, nurse practitioners, and the Medical Director as needed.

? Ensure Primary Care Physicians are aware of patients' health conditions and progress.

? Provide health education to patients.
,
? Work with patients in the prevention of illness, promotion of health, and management of chronic health problems.

? Provide referrals both internally and externally to appropriate medical resources.

? Engage with providers and patients to create a trusting environment.

? Maintain a safe, efficient, and pleasant environment conducive to optimum patient wellbeing.

? Function in a collaborative relationship with physicians and other healthcare professionals in the clinical management of patients.

? Maintain good working relationships with other personnel, medical staff, and administration.

? Report suspicious, unethical, or illegal conduct and cooperate with approved investigations and inspections.

? Perform stock room inventory regularly and maintain medications, equipment, and supplies appropriately stocked.

? Develop an understanding of Health Plan's encounter data submission process and CMS requirements.

? Participate in grievance, audit, continuing education, quality assurance, and quality improvement programs.

? Serve as an ongoing resource to providers and case management teams.

? Serve on and participate in appropriate clinical committees as assigned.

? Provide clinical leadership of teams, consisting of social workers and peer navigators, and function as a liaison to the overseeing physician and psychiatrist.

? Monitor, train, and coach patients on how to live with chronic conditions to ensure health goals are met.

? Collaborate with SCAN, Healthcare in Action team, and Quality Improvement team to achieve corporate goals.

? Function as part of the care team, with advanced knowledge, skill, and expertise in history taking, physical exams, treatment, and clinical care management of patients.

? Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, and participating in professional societies.
Requirements
? Completed LVN training, required.
,
? Current Licensed Vocational Nurse is required.
,
? 2+ years of direct patient care experience working in a medical office or hospital setting.
,
? Understanding of ICD-10 Coding preferred.
,
? Excellent assessment skills required.
,
? Ability to problem-solve and prioritize changing patient conditions required.
,
? Knowledge of efficient interviewing practices and techniques used to gather information.
,
? Strong interpersonal skills with the ability to build relationships with providers and patients.

? Excellent verbal, written communication, and listening skills both in-person and on the phone required.

? Ability to work on a team and maintain good working relationships with staff and patients required.

? Excellent customer service skills required.

? Ability to be culturally sensitive and work with a diverse, geriatric patient population.
