Commonspace - HOME Care Coordinator
(HOME: Housing Outreach & Member Engagement Program)
Position Summary:
Commonspace HOME program is a team-based model of care that supports community members who are experiencing long-term homelessness. The program team comprises of housing navigators, peer support specialists, care coordinators, and a clinician. Program participants benefit from the team?s unique roles to create and guide their own plans on housing, employment, income benefits, mental and physical health, substance use, or crisis planning. Program support services continue to be available following transitions into housing or other identified goals. Our HOME program empowers participants to identify, lead and direct their goals with the team?s support.
Responsibilities: HOME Housing Navigator coordinates and supports HOME participants in gaining and maintaining access to services to meet the goals of the participants? plan of care.
OVERVIEW OF RESPOSIBILITIES
? Build and maintain mutual, genuine relationships with participants.
? Collaborate with program team to build a shared network and program vision
? Engage in outreach to participants to cultivate consistent connection to support progress in housing search and connection to mainstream services.
? Conduct program assessments (SPDAT), intake paperwork, Plan of Care, and monthly documentation.
? Support and empower participants to develop and strengthen their ability to effectively navigate complex systems for themselves and their families.
? Assist participants in completing applications for available housing vouchers and affordable housing programs.
? Assist participants in navigating services related to their individual plan of care goals.
DETAILED RESPOSIBILITIES
Provide support to participants in the process of moving into and maintaining housing.
? Coordinate between property owners/management, relevant housing authorities/nonprofits, and participants during and after unit lease-up and move-in.
? Support participants in moving into housing, including but not limited to obtaining furnishings, making the new apartment feel like home, and offering any needed support through this transition.
? Collaborate with the other program team members (peer support, clinician, housing navigator) to offer coordinated support to participants following successful housing placement.
? Maintain knowledge of and support participants in connecting with the community resources they identify they need for continued housing stability.
Provide support to participants in accessing and maintaining services to meet goals of individualized plan of care.
? Assist participant(s) in coordinating their medical, mental health, recovery services, and other related goals.
? Cultivate and maintain working relationships with community primary care providers, behavioral health providers, inpatient facilities, emergency departments, residential facilities, crisis services, shelters and correctional facilities for HOME participants care.
Collaborate with HOME Team lead and Clinician to ensure ongoing program compliance.
? Complete and maintain all required documentation for HOME enrollment with participants and clinician, including: SPDAT, intake forms, plan of care.
? Maintain monthly documentation in electronic health record of services provided to participants.
? Collaborate with the administrative coordinator and Team Lead in collecting and reporting on all necessary documentation required for ongoing program compliance.
? Engage in regular supervision with HOME Team Lead.
Working collaboratively within Commonspace programs and community as a representative of the agency:
? Collaborate with staff of various other Commonspace programs to offer goal-related support to participants as needed.
? Collaborate with agency staff on joint agency projects and initiatives as needed.
? Demonstrate commitment to cultural sensitivity and the ability to work effectively with diverse populations.
? Maintain knowledge of and compliance with Commonspace?s policies, procedures, and legal obligations.
? Positively represent Commonspace in the community.
? Actively participate in, assist, and/or facilitate agency activities and initiatives.
Required Education, Skills and Experience:
? Attention to detail and time management with documentation.
? Relationship development: ability to develop genuine, mutual and trusting relationships with diverse groups of people.
? Experience with Federal, State, and local housing and assistance programs.
? Experience with Mainecare billing programs.
? Experience in maintaining organization while moving through a dynamic, multi-step process.
? Excellent verbal and written communication skills.
Preferred Education, Skills and Experience:
? Bachelor?s Degree in human services, social work or related field of study; or comparable combination of education and relevant experience.
? Experience supporting people to achieve their goals within traditional, complex, and/or bureaucratic systems that normally marginalize them.
? Experience successfully supporting people through the process of obtaining and maintaining permanent, stable housing.
? Lived experience with any combination of: homelessness, poverty, substance use disorder, recovery, incarceration, and mental health challenges.
? Interest in growing in one of the following areas of housing support and navigation work: landlord engagement, street outreach, and/or housing stabilization.
Certification & Licensure
? Valid Driver?s License and personal vehicle for work use. ?Required
? CPR & First Aid ? Preferred
Role Presence
? This is a Full-Time Hybrid Position. This position?s schedule is between 9a and 5p, Monday through Friday, with a combination of working hours in the office, in the community, and at home.
Physical Requirements
Some of Commonspace?s requirements include:
? Must be able to sit, stand, and walk for extended periods of time.
? Must be able to ambulate up and down stairs.
? Must be able to lift 15 pounds as necessary.
Working Environment
This position requires working with individuals who may be under the influence of substances, exhibiting mental health symptoms, or present challenging behaviors. Medical and other emergencies may occur in the working environments, including substance overdoses, suicidal ideation and/or self-harm, physical and/or verbal violence; when such events occur contacting emergency services for assistance is essential and required.
