About the position

The Community Health Advocate, Experienced will play a crucial role in supporting Blue Shield of California Promise Health Plan members by conducting outreach and assessments focused on social drivers of health needs. This position requires a proactive approach to identify resource gaps in both medical and community services, and to support the implementation of new services aimed at closing these gaps. The advocate will work closely with moderate-risk members, assessing their psychosocial and social health needs, and documenting the results in appropriate systems. This role involves establishing partnerships with community organizations, health plans, and providers, and participating in community engagement activities with local agencies such as faith-based organizations, community centers, and government agencies. In addition to conducting assessments, the Community Health Advocate will assist members in navigating the healthcare system by problem-solving barriers to access. This includes visiting members at their homes, accompanying them to medical appointments, and helping them complete necessary forms to access services. Building rapport and establishing trusting relationships with members and their families is essential, as is the ability to identify social drivers of health concerns and develop plans to address disparities in care. The advocate will also be responsible for documenting member updates, submitting timely reports, and making necessary follow-ups on mandated reporting, such as Adult Protective Services and Child Protective Services. Other duties may be assigned as required. Responsibilities
? Conduct outreach to Blue Shield of California Promise Health Plan members and perform assessments for social drivers of health needs.

? Identify resource gaps in medical and community services and support the implementation of new services to close these gaps.

? Assess psychosocial and social drivers of health needs for moderate-risk members and document assessment results in appropriate systems.
,
? Establish relationships and partner with community organizations, health plans, and providers by participating in community engagement activities.
,
? Assist members with moderate health conditions in problem-solving barriers by identifying, locating, connecting to, and navigating needed community and medical system services.
,
? Visit members at their homes and accompany them to medical appointments as needed.
,
? Help members complete forms to access needed services.
,
? Build rapport and establish trusting relationships with members and their family support systems.
,
? Identify social drivers of health concerns and develop a plan to address social and health disparities in accessing appropriate care and services.
,
? Document member updates and progress notes in appropriate systems and submit timely reports with recommendations for improved member outcomes.

? File and follow up on mandated reporting, such as Adult Protective Services and Child Protective Services.

? Perform other duties as required.

Requirements
? Bachelor's Degree in Social Work or a relevant field.
,
? At least 3 years of prior relevant experience working with populations impacted by health and social issues.

? Proficient in Microsoft Excel, Outlook, Word, and PowerPoint.
,
? Minimum of 1 year experience navigating, linking to, and interacting with community-based organizations and governmental systems.

? Valid CA Driver's License and proof of auto insurance coverage required.

? Excellent communication skills, both orally and in writing.

? Knowledge of health disparities in diverse communities.
,
? Effective time management skills and ability to prioritize multiple responsibilities.

? Experience working with Medi-Cal populations preferred.

? Bilingual/Spanish speaking preferred.
