Primary Purpose of Position: The Clinical Pharmacist is a collaborative position with all clinical departments within Partners and with community stakeholders. This position will oversee and perform pharmacy clinical services management and provide for population health data management integration activities as it relates to risk stratification and the Tailored Plan member care coordination and provision of high quality cost effective healthcare for members. Lastly, this position will serve as a subject matter expert in pharmacotherapy and population health around pharmacy and medical claims data utilization to improve medication use.

Role and Responsibilities: (in order of priority)

Medication Reconciliation/Medication Optimization:
? Refine Partners process for and actively participate in medication reconciliation for targeted members for the detection and reduction medication discrepancies and to assure positioning for adherence in the community.
? Medication reconciliation is targeted at post-hospital discharge setting for patients at elevated risk of re-hospitalization.
? Participate in continued evaluation of the effects of medication reconciliation on therapeutic and economic outcomes, especially during patient transitions across care settings.
? Establish and maintain connectivity with state hospital transitional care teams; targeted patients leaving these facilities will be connected with community pharmacy in home community and thus supported by trained clinical pharmacists to perform medication reconciliation, provide medication adherence support, and communication of key clinical information to the community care providers, in order to decrease rehospitalization and ED use.
? Identify patients who are candidates for clozapine, LAI, and MAT and operationalize/facilitate these care strategies at the practice level; identify patients who would benefit from ACT team support.
? Monitor patient response to treatment, drug/drug and drug/disease interactions; assess medications prescribed at dosages higher that approved by the FDA for specific indications, and side effects assessment.
? Follow up with patients who meet criteria for non-adherence intervention/supports and problem solve to improve.

Collaborative Team-Based Care Participation:
? Focus on enhancing predictive analytics, patient risk stratification instruments to clearly identify target population for focus of specific medication management services; assists with the development of a construct to facilitate data review, identification, and analysis of priority patient/high risk patient lists/data sets to better position management of target populations.
? Work with care management and other clinical services departments to facilitate access to whole person care services related to medication management. Provide clinical pharmacy input, with whole person care emphasis, during the preparation of interdisciplinary care plans for patients
? Propose projects/initiatives based on data driven identification of issues (recidivists, frequent ED use, medication non-adherence, failure to keep follow-up appointments) that may be more effectively managed to prevent/minimize psychiatric relapse.
? Identify and connect high risk/high acuity medication non-adherent patients with community pharmacy in member home community for medication adherence coaching and supports
? Assist in the selection of the most cost effective, evidence-based behavioral health regimen for a particular patient; monitor patients for appropriate lab and recommended follow-up.
? Identify individuals with recent decrease in functional status through data analytics and work collaboratively with care coordination staff to address concerns related to medication management.
? Coordinate/participate in activities related to improving outcomes for targeted disease management initiatives leading to improved health (such as mental health/developmental disabilities.)

SME/Clinical Consultation/Support:
? Serve as Subject Matter Expert and clinical resource in the arena of whole person care drug information and therapeutics for the Partners Health Management clinical teams as well as for plan catchment area behavioral health teams, providers, and support staff (evidence-based literature, curbside consults, complex case review).
? Work collaboratively with Partners Behavioral Health Management to develop, identify, then disseminate standardized methods to optimize pharmacotherapy as one aspect of wrap-around treatment plan; assist with education around the appropriate use of patient psychometric screening and/or assessment instruments/outcome measures to improve screening and/or measurement-based care and others to identify those needing additional referral and/or guidance for behavioral health issues.
? Provide SME consultation as needed for Partners Health Management care management and other clinical personnel regarding improvement strategies for outcomes directed patient focused medication management.
? Provide education to Partners staff and stakeholders on improved medication adherence and therapeutic outcomes in patients with chronic conditions.
? Provide highly specialized education and training support for other professional services in the broad area of advanced pharmacotherapy, with emphasis on psychiatry issues.
? Provide consultation/outreach/education with community pharmacists and other community stakeholders regarding Partners clinical initiatives for targeted members to ensure success of initiative.

Pharmacy Benefits Management (PBM):
? Serve as SME/pharmacy team member in use of PBM clinical resources to achieve objectives noted above.
? Serve as pharmacy/medical SME in assisting with developing clinical policies and procedures related to and/or supporting PBM and affiliated pharmacy benefits management activities.
? Support Partners programs to address statewide healthcare initiatives such as AKIDS, ASAP, Narcotic Lock-in, and Opioid Stewardship programs.

Reporting/Outcomes:
? Consultative activities documented and uploaded as required by Partners operational procedures. Quality Improvement activities reporting provided in relevant cross functional team.
? In conjunction with Partners Health Management clinical and administrative leadership, works to align Partners key performance measures with those of DHHS and which will support fidelity to federal health home quality measures; including development/implementation of standard processes, quality measurement and performance measurement as well as follow-up as indicated by analytics (PDCA process).
? Evaluate how collaborative care models improve medication use measures, and how these configurations affect current and downstream healthcare costs.
? Assures data is tracked and all reporting and documentation requirements are met relative to clinical pharmacy operations.

Collaboration and Consultation:
? Participation in clinical and operations meetings as required for effective communication/planning/execution of key deliverables.
? Active participation in integrated care meetings including UM clinical rounds, Pharmacy and Therapeutics/Drug Utilization review committee and quality improvement teams as requested.

Performs related tasks as required and/or needed.

Knowledge, Skills and Abilities:
? Extensive understanding of the Diagnostic and Statistical Manual of Mental Disorders (DSM- 5).
? Knowledge of best clinical care practices for individuals meeting Tailored Plan member criteria; ability to research and analyze the medical literature to identify evidence-based practices in areas of drug information/utilization, disease states, and clinical practice guidelines.
? Exceptional interpersonal skills and be able to communicate effectively with individuals/families supported, service providers, community agencies, other employees and a variety of other community members.
? Problem solving, critical thinking, analytical skills, and conflict resolution skills are essential to balance the needs of both internal and external customers.
? Good written and verbal communication skills; demonstrated ability to integrate with the healthcare team.
? Detail-oriented, able to organize multiple tasks and priorities and to effectively manage projects from start to finish.
? Ability to communicate by telephone or other electronic device when working in the community or remotely/virtually.
? Ability to work independently, exercising sound judgment, discretion and the initiative to facilitate change.
? Excellent organizational skills, diplomacy, and professionalism.
? Exhibit sensitivities to cultural differences present in service populations.
? Physical demands include: frequent movement throughout the community, periods of standing, sitting and walking; reaching and light lifting; finger dexterity; visual and auditory acuity within normal limits.
? Must be proficient in electronic health records and various health care documentation platforms such as the NC Immunization Registry, NC Controlled Substance Reporting System, etc.
? Responsible for maintaining patient and family confidentiality in accordance with HIPAA.
? Ability to operate a personal computer and working knowledge of word processing, spreadsheet, and database software applications; proficiency in using database information systems and Microsoft Office Suite.
? Extensive knowledge of pharmacy and its related sub disciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug use review, quality assurance, legal, regulatory, and standards of practice.
? Demonstrated expertise in performance improvement practices and associated project management.

Education/Experience Required:
? Doctor of Pharmacy Degree (PharmD) from an accredited College of Pharmacy, clinical pharmacy residency or equivalent experience and licensure by North Carolina Board of Pharmacy.
? Minimum 4 years of clinical pharmacy experience with direct patient care focus, along with involvement in clinical policy development, formulary development, drug use evaluation, and quality assurance activities.

Education and Experience Preferred:
? Pharmacy Board certification (Board of Pharmacy Specialists) in psychiatric pharmacy, ambulatory care pharmacy, or general medicine preferred.
? PGY-1 residency or equivalent clinical experience (4 years in clinical practice with at least 50% of time spent in clinical activities)
? Medicaid pharmacy benefit management experience, including managed care environment with involvement in clinical policy development, formulary development, drug use evaluation, and quality assurance activities. Psychiatric pharmacy experience preferred.

Licensure/Certification Requirements:

Current unrestricted licensure with the NC Pharmacy Board. Employee is responsible for complying with respective licensure board's continuing education/training requirements in order to maintain an active license.
