Job Overview
We are on the lookout for an experienced RN to deliver high-quality, compassionate care for patients in their homes. Key Responsibilities
? Conduct patient assessments and collaborate with the care team to craft and implement tailored care plans.
? Refer patients to other hospice care disciplines as necessary, or provide justification for not doing so.
? Maintain accurate and timely documentation of patient care progress in accordance with established policies.
? Record changes and outcomes related to pain and symptom management as appropriate.
? Educate patients, families, and caregivers on effective patient care techniques.
? Maintain regular communication with the clinical manager and care team.
? Fulfill on-call responsibilities and provide services to patients and families as assigned.
? Undertake additional duties as needed.
Qualifications
? A minimum of one year of nursing experience.
? A current RN license in the state of Wisconsin.
? Current CPR certification.
? A valid driver?s license, reliable transportation, and liability insurance.
Compensation And Benefits
? Competitive Pay: (88,000 - )92,000 per year, plus a $10,000 sign-on bonus and mileage reimbursement.
