About the position

DaVita Integrated Kidney Care (IKC) is seeking a Full-Time RN Case Manager to join our team in the East Point, GA region. This position is designed for a dedicated Registered Nurse (RN) who will support patients with complex health needs, particularly those with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD). As an RN Case Manager, you will play a crucial role in helping patients navigate the healthcare system, ensuring they receive the necessary medical, social, emotional, and financial support. Your responsibilities will include conducting medical record reviews, identifying patient needs, and implementing appropriate interventions to enhance patient care. In this hybrid-remote role, you will work from home while also traveling within your assigned geography for in-person visits to dialysis clinics and nephrology practices. The position requires a Monday to Friday schedule, with flexibility to accommodate patient and nephrology partner availability. You will be responsible for quickly building empathetic relationships with patients and their families, coordinating care transitions, managing ongoing needs, and establishing treatment plans in collaboration with the healthcare team. Additionally, you will address social determinants of health, ensuring that all aspects of a patient's well-being are considered in their care plan. Your role will also involve assessing patients to identify any unreported medical conditions or changes that could lead to adverse outcomes. This position is not only about providing care but also about advocating for patients and ensuring they receive comprehensive support throughout their healthcare journey. Responsibilities
? Support patients with complex health needs, particularly those with CKD and ESKD.

? Conduct medical record reviews and consultations to identify patient needs.

? Implement appropriate interventions based on identified needs.

? Work autonomously and collaboratively with the healthcare team to coordinate care.

? Build empathetic relationships with patients and families.

? Identify and address social determinants of health affecting patients.

? Assess patients to identify unreported medical conditions or changes.

Requirements
? Current RN License is required; BSN preferred.

? Minimum of 2 years RN experience required with healthcare performance coaching.
,
? Current CPR or BLS certification is required.
,
? Current driver's license and willingness to travel within an assigned territory.
,
? Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required.
,
? Home office with internet connectivity at a minimum of 1MB upload and 1MB download speed required.

Nice-to-haves
? 2+ years of Case Management or Chronic Care Management experience.

? 1+ years of utilization management experience.

? Spanish-speaking preferred in some markets.
