As a Non-Premium Oncology Nurse Case Manager at Cigna, you will play a crucial role in advocating for our customers by coordinating, supporting, and guiding their care throughout their healthcare journey. This position requires a compassionate and proactive approach to ensure that customers, their families, and caregivers receive the necessary support to navigate complex healthcare systems. You will be responsible for embodying and promoting the Care Solutions cultural beliefs, which include delivering exceptional customer experiences, fostering collaboration, taking ownership of your responsibilities, adapting to changing needs, being innovative in problem-solving, and prioritizing self-care and compassion in your interactions with colleagues and customers alike. In your daily responsibilities, you will collaborate with customers to create personalized care plans, documenting these plans in our medical management system. You will work closely with customers to establish their health goals and interventions, utilizing motivational interviewing techniques and shared decision-making to empower them in achieving optimal health outcomes. Your role will involve regular communication with customers, their families, and healthcare providers through various channels, including phone, email, text, and chat, ensuring that all parties are aligned and informed about the care process. You will also collaborate with a multidisciplinary team, including nutritionists, pharmacists, behavioral clinicians, and medical directors, to provide comprehensive support that addresses the whole person?s health needs. Tracking daily activities and outcomes will be essential to identify trends and improve service delivery. You will be expected to adhere to standard operating procedures, manage multiple systems and applications, and stay informed about health promotion and disease prevention topics relevant to your customers. Continuous education and maintaining clinical expertise will be vital components of your role, ensuring that you remain a knowledgeable resource for your customers and colleagues.
Responsibilities
? Collaborates with customer in creation of care plan and documents plan in medical management system.

? Partners with each customer to establish goals and interventions to meet the customer's needs.

? Establishes plan of care in conjunction with the customer and provider then documents into a medical management system.

? Utilizes motivational interviewing, behavior change, and shared decision making to help customers achieve optimal health and well-being.

? Empowers customers with skills to enhance interaction with their providers.
,
? Interfaces with the customer, family members/caregivers, providers, and internal partners to coordinate the needs of the customer through telephonic, email, text, and chat interactions.

? Collaborates with nutritionist, pharmacist, behavioral clinician, Medical Director and customer's provider and other Cigna Medical Management programs to provide whole-person health support.

? Tracks daily activities to trend volume and outcomes.

? Follows standard operating procedures.
,
? Toggles between multiple systems and applications.
,
? Research relevant topics in health promotion and disease prevention, as required for specific customers.

? Prioritizes work to meet commitments aligned with organizational goals.

? Understands and adheres to Case Management performance measures to deliver on key results.

? Completes training within the communicated time limit as required per role.

? Demonstrates evidence of continuing education to maintain clinical expertise and certification as appropriate.
Requirements
? Active unrestricted Registered Nurse (RN) license in state or territory of the United States.

? Minimum of two years full-time direct patient care as an RN required.
Nice-to-haves
? Experience in the specialty Oncology preferred.

? Must have an active and unencumbered RN License.

? Within four (4) years of hire as a case manager will possess a URAC-recognized certification in case management.

? Excellent communication skills including telephonic (verbal) and digital (messaging, emails).

? Skilled in clinical acumen to form a judgement and act.

? Strong computer skills in Microsoft Word, Excel, Outlook, and ability to perform thorough internet research.
