Bilingual Care Coordination Assistant (Remote/Hybrid, Must be LPN or MA)

What to Expect (Job Responsibilities):
• Assist in implementing care gap closure and support providers in offering evidence-based medicine
• Manage disease registries and provide patient and family self-management support
• Collaborate with provider care teams and community services to ensure coordinated care
• Document and access information effectively within multiple electronic health records and health-related databases
• Function autonomously while being accountable for patient care outcomes

What is Required (Qualifications):
• Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) required
• High school diploma required; Bachelor's Degree preferred
• Minimum of five years' experience in a healthcare setting, preferably in an ambulatory setting
• Moderate to high computer literacy, including knowledge of Microsoft Office suite software
• Demonstrated customer-focused interpersonal skills for effective interaction with diverse populations

How to Stand Out (Preferred Qualifications):
• Knowledge of performance improvement, billing, coding, and documentation requirements
• Familiarity with evidence-based behavioral change methodologies and motivational coaching techniques
• Experience working in a shared leadership environment
