About the position

The RN Care Coordinator role at UnitedHealthcare is a vital position within an Integrated Care Model, focusing on advocating for members' needs and ensuring they receive appropriate care services. Responsibilities
? Assess, plan, and implement individualized care strategies for members.

? Identify and initiate referrals for member transitions to community services and assist with social service programs.

? Manage the care plan throughout the continuum of care as a single point of contact.

? Communicate with stakeholders to ensure quality coordinated care and services are provided promptly.

? Advocate for members and families to ensure their needs and choices are represented and supported by the healthcare team.

? Visit Medicaid members in their homes and other settings, including community centers and hospitals.

Requirements
? Current, unrestricted RN license in the state of Texas.

? 2+ years of experience working within the community health setting or in a healthcare role.

? Intermediate level of computer proficiency, including Microsoft Outlook and Teams.

? Reliable transportation and ability to travel within the assigned region.
,
? Access to a designated quiet workspace at home with the ability to secure Protected Health Information (PHI).
,
? Reside in a location with access to a UnitedHealth Group approved high-speed internet connection.

Nice-to-haves
? 1+ years of experience with long-term care services and support, Medicaid or Medicare.

? Ability to create, edit, save, and send documents, spreadsheets, and emails.

? Knowledge of integrated settings principles, including federal and state requirements.
