About the position

The Behavioral Health Care Manager at UnitedHealthcare will serve as the primary care manager for members with complex behavioral needs. This role focuses on care management and coordination activities that support members' medical, behavioral, and socioeconomic needs, promoting appropriate service utilization and improved quality of care. The position requires collaboration with the provider community to reduce fragmentation in care and to promote health equity, addressing health disparities through a comprehensive approach.

Responsibilities
? Serve as primary care manager for members with high behavioral needs.

? Engage members through various modalities to complete comprehensive needs assessments.
,
? Develop and implement individualized, person-centered care plans.
,
? Collaborate with internal care teams, providers, and community resources to meet care plan needs.
,
? Monitor and update care plans based on member feedback and progress.
,
? Assist members in developing personal wellness and health crisis plans.
,
? Provide psycho-education regarding conditions, medications, and healthcare system utilization.
,
? Offer coaching and supportive counseling for self-management of care needs.
,
? Facilitate referrals and linkages to appropriate services and resources.
,
? Perform activities to address HEDIS/STAR gap closure.
,
? Support proactive discharge planning after ER or inpatient admissions.
,
? Educate members about Medicaid benefits and coordinate care needs.
,
? Document all care management activities in the clinical care management system.
,
? Coordinate care management activities with a broader team to promote team-based care.
,
? Provide guidance and consultation to team members regarding behavioral health conditions.

Requirements
? Must possess an MSW or a Master's degree in a behavioral health-related field with a current, unrestricted license (e.g., LCSW, LPC).
,
? BA in behavioral health field with 5+ years of direct clinician experience.
,
? LPN with 5+ years of direct experience serving individuals with behavioral health conditions.
,
? 2+ years of experience directly serving individuals with behavioral health conditions.
,
? 2+ years of experience serving Medicaid populations.
,
? 1+ years of field-based work experience in community settings.
,
? Familiarity with state-specific behavioral health Medicaid benefits.
,
? Ability to travel 25% of the time for the role.
,
? Access to a designated quiet workspace at home with high-speed internet.

Nice-to-haves
? Experience serving individuals with co-occurring disorders.
,
? Experience working in team-based care.
,
? Training or certifications in Motivational Interviewing, Trauma-Informed Care, or Person-Centered Care.

? Bilingual in Spanish or other relevant languages.

? Experience working in Managed Care.

? Lived experience in managing chronic health or behavioral conditions.
