What to Expect (Job Responsibilities):
• Provide resource support to members and connect them to local resources for various services
• Assist in assessing member needs and developing care plans/service plans
• Connect members to local resources, obtain required documentation, and support the application process
• Monitor delivery of care/services and perform follow-ups to ensure timely access
• Work with providers and community stakeholders to support members in accessing essential care/services

What is Required (Qualifications):
• Bachelor's degree and 2 ? 4 years of related experience
• Ability to develop, assess, and adjust care plans
• Excellent coordination and communication skills
• Active participation in team care management rounds
• Risk identification and management skills

How to Stand Out (Preferred Qualifications):
• Experience in a similar role in a healthcare or social services setting
