Job Overview
We are actively seeking dedicated and professional Remote Healthcare Consultants to enhance our team. In this pivotal role, you will oversee case management activities for enrollees in our Long Term Care/Comprehensive Program, ensuring favorable healthcare outcomes through effective care coordination, support, and education.
Key Responsibilities
? Employ critical thinking and sound judgment to aid in the case management process.
? Perform thorough evaluations of members utilizing care management tools and data insights.
? Coordinate and execute assigned care plan activities, vigilantly monitoring progress.
? Facilitate multidisciplinary reviews to attain optimal outcomes for our members.
? Identify and elevate quality of care concerns through specified channels.
? Leverage negotiation and motivational interviewing techniques to secure appropriate services and encourage lifestyle modifications.
? Provide guidance, information, and support to empower members in making autonomous health decisions.
? Assist members in actively collaborating with their healthcare providers in decision-making.
? Adhere to case management and quality management processes in alignment with regulatory and accreditation standards.
Required Skills
? Bilingual proficiency in Spanish and English.
? Strong communication skills, both verbal and written.
? Proficiency in Microsoft Office, particularly Excel.
? Exceptional critical thinking and judgment abilities.
? Capable of multitasking and prioritizing effectively in a dynamic environment.
? Skilled in negotiation and motivational interviewing.
Qualifications
? Bachelor's degree in Social Work or a closely related field (nursing degrees will not be accepted).
? Demonstrated case management experience, ideally with a background in long-term care.
? Willingness to travel approximately 75% within the region to engage members in their homes, assisted living facilities, and nursing homes.
? Must reside in Miami-Dade County, FL.
