Quality Care Manager (Medical Assistant, LPN)
Posted 2025-03-15The Quality Care Manager is responsible for ensuring a successful Quality Care Management (QCM) program and for providing a positive, supportive environment for patients and other staff while working in conjunction with physicians, advanced practice clinicians, front office and clinical staff. A successful Quality Care Manager will have strong communication skills and dedication to customer service and patient care. This position is responsible for the resolution of a broad range of administrative, clinical and business operations associated with this program. The incumbent is tasked with supporting the QCM program to assist in strategies which are created to improve provider practice initiatives that support optimal health care delivery-model execution and performance.
Summary of Essential Job Functions:
? Reviews and completes initial engagement setup of all patients enrolled in QCM program to include data verification of demographics, documentation and provide instruction and guidance to program.
? Completes initial quality assurance review during first call and monthly thereafter.
? Performs data monitoring of RPM program daily Monday- Friday.
? Follows Interactive Communication Protocol.
? Follows Provider Notification Protocol.
? Maintains proper, concise and completed EMR documentation and documentation for all QCM program processes.
? Works with front-line clinical staff to map care team processes and workflow improvements.
? Analyzes transmission data for verification and integrity.
? Develops training materials, manuals and provider training curriculum.
? Conducts presentations to staff, providers and members of committees and other departments to enhance the QCM program.
Minimum Requirements: ? Clinical degree e.g. MA, LPN, or RN degree required. ? Must maintain current certification from the Certifying Board of the American Association of Medical Assistants (AAMA) or current nursing license, in good standing with the State licensing board. ? 3-5 years? experience in Health Care, Quality, IT or related field. ? Familiarity with current applicable laws and regulations, including federal, state, and non-federal regulations surrounding health care, including HIPAA, HITECH, ACA and other regulations. ? Strong verbal and written interpersonal skills as well as presentation and teamwork skills. ? Strong analytical skills. ? Excellent time management and organization skills. ? Intermediate to advanced ability to utilize multiple software applications, including but not limited to: MS Excel, Word, Access, PowerPoint and various EHR systems. ? Experience in medical billing, electronic health records or practice management technology is desired.
