Job Overview

We are in search of a compassionate and committed healthcare professional to join our esteemed team as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). In this pivotal role, you will be responsible for developing and managing patient care plans that cater to the distinct needs of each individual. This position offers you the opportunity to conduct patient... interviews, document their medical histories, and monitor their overall physical health, all while ensuring that patients receive the regular care they deserve.

Key Responsibilities
Deliver personalized, one-on-one care for clients, ensuring adherence to individualized care plans that uphold their dignity and promote independence.
Monitor clients vital signs and report any irregularities to the case manager and physician promptly.
Administer medications, assist with daily feeding, provide oxygen therapy, and perform ostomy care as necessary, in line with the client's care plan and ongoing evaluations.
Collaborate effectively with a multidisciplinary team of healthcare professionals, including Physical, Occupational, and Speech Therapists, to implement established care protocols.
Adhere to stringent infection control measures, promptly reporting any potential communicable diseases or infections and ensuring compliance with established procedures.
Maintain precise records of all nursing actions and care provided, facilitating open communication with family members, case managers, and physicians. Inform them of any changes in the client s medical condition and needs.
Manage the ordering of essential supplies and medications to ensure optimal stock levels.
Ensure the fulfillment of all patient needs as outlined by physician directives.
Monitor incidents involving clients and ensure accurate and timely completion of incident reports, keeping case managers informed and assessing clients post-incident.
Maintain all medical equipment, reporting any malfunctions to Durable Medical Equipment for necessary repairs.
Execute additional duties as assigned.

Required Skills
Comprehensive understanding of individual service plans, with a focus on client rights and health needs.
Superior interpersonal skills, showcasing strong customer service, patience, empathy, and compassion.
In-depth knowledge of patient care policies and regulatory requirements.
Familiarity with various medical conditions, including their symptoms and indicators of improvement.

Qualifications
Successful completion of post-secondary coursework required for licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
Currently possess an active license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing in the state(s) of practice.
Valid CPR certification.
