Remote Care Coordinator (LPN)

Your Role
? Provide monthly calls and support to patients enrolled in the CCM program.
? Utilize your nursing skills to educate patients and their families.
? Monitor patients progress and work with the physicians to ensure quality patient care.

Qualifications
? Active LPN license (compact)
? Experience in geriatrics or primary care
? Ability to sit and talk on the phone for long periods of time
? System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
? Must live and work full time in the US - you can work from anywhere within the US but not outside.

The Pay
? The LPN pay ranges from $21.85 - $23.77 per hour (40hrs/week)
? The pay is a base pay of $17.50/hr plus a bonus of $2.00 per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
