About the position

The Lung Nodule Nurse Navigator at AdventHealth plays a crucial role in ensuring that cancer patients navigate the healthcare system effectively and efficiently. Under the guidance of the Thoracic Program Leader, this position is responsible for providing comprehensive support to patients diagnosed with lung nodules, ensuring they receive appropriate physician care, educational resources, and support services throughout their healthcare journey. The Nurse Navigator will establish and maintain strategic relationships with community physicians and their office staff, facilitating seamless communication and collaboration to enhance patient care. In this role, the Nurse Navigator will manage and follow up on incidental lung nodule referrals, particularly those identified through the Epic dashboard and lung cancer screenings. This includes facilitating continuity of care between various hospital departments and providing timely follow-up for laboratory, radiology, and pathology procedures. The Nurse Navigator will be proactive in identifying and resolving issues related to patient healthcare delivery and physician relations, ensuring that patients receive the highest quality of care. Additionally, the Nurse Navigator will coordinate and participate in weekly thoracic tumor board meetings and lung nodule clinics, tracking scheduled procedures, following up on pathology results, and monitoring the outcomes of these meetings. Collaboration with ordering providers is essential to coordinate additional workups for patients based on their treatment plans, including necessary diagnostic procedures and referrals. The Nurse Navigator will adhere to the Oncology Lung Navigator Workflow Standard Operating Procedure to maintain consistency and quality in patient care.

Responsibilities

Requirements
? Graduate of a school of Nursing.
,
? Current valid State of Florida license as a Registered Nurse.
,
? Current valid Driver's license.
,
? Basic Life Support (BLS) certification.
,
? Minimum of one year Oncology and/or thoracic experience.

Nice-to-haves
? Bachelor's degree (BSN) or Master's degree (MSN) in Nursing.

? Current certificate in Oncology nursing.
