Job Overview
Join our dedicated team as a Case Manager Level II, where you will be instrumental in delivering vital post-release services to children and their sponsors. This remote position offers you the chance to utilize your expertise in trauma-informed care and child welfare practices while ensuring every child receives the essential support they need for a successful transition.
Key Responsibilities
? Manage a diverse caseload of post-release services, ensuring adherence to policies and mandated timelines.
? Conduct comprehensive assessments of children and their sponsors, validating the suitability of placements and living conditions.
? Collaborate with care providers to identify and address additional needs while providing necessary resources.
? Deliver ongoing case management services through both virtual and in-person engagements, applying trauma-focused strategies to effectively engage children and sponsors.
? Provide psychoeducation, therapeutic techniques, and sustained support to children and their sponsors.
? Maintain precise, confidential documentation in accordance with program regulations, safeguarding sensitive information.
? Act as a mandated reporter, addressing any concerns related to abuse or neglect and escalating issues to the appropriate authorities when necessary.
? Participate in training, meetings, and quality improvement initiatives as required.
? Travel occasionally within the U.S. for home studies and post-release visits, with potential evening and weekend hours.
Required Skills
? Strong interpersonal and communication abilities, facilitating effective connections with families and stakeholders.
? Fluency in both English and Spanish (verbal and written).
? Proficiency in Microsoft Office and other data entry systems.
? Knowledge of family preservation, kinship care, child welfare policies, and relevant state/federal regulations.
? Capacity to manage sensitive situations and make thoughtful decisions under pressure.
Qualifications
? Bachelor's Degree in Social Work (BSW) or a related field such as psychology, sociology, education, or behavioral sciences.
? Minimum of 1 year of experience in case management, ideally in trauma-informed care and working with immigrant populations.
? Experience in drafting professional assessment reports and delivering trauma-informed interventions for youth.
? A valid driver's license and the ability to travel as required.
