The Community Health Navigator position is a vital role within the Maryland government programs division, designed to serve as a bridge between members, clinical staff, and administrative personnel. This position is focused on helping members, families, and caregivers navigate the healthcare delivery system effectively. The Navigator will engage in outreach efforts, provide education, and offer follow-up and coordination support to ensure that members have access to the necessary resources and information for their healthcare needs. The role requires a combination of telephonic outreach and community-based engagement, including home visits to members, to foster a supportive healthcare environment. In this hybrid role, the Community Health Navigator will primarily work remotely within Baltimore and its surrounding areas, but will also be required to attend meetings and training sessions at CareFirst locations periodically. The Navigator will actively participate in community events, representing CareFirst and its commitment to promoting health and wellness within the community. This position is essential for ensuring that members are connected with appropriate care channels, such as pharmacy services, insurance, billing, and referrals, thereby facilitating a seamless healthcare experience. The Navigator will also be responsible for following up with patients between care episodes to ensure they attend their appointments and receive the necessary support for transportation and pharmacy needs. Collecting and relaying information to the Care Coordination team is a critical function of this role, ensuring that all relevant data is communicated effectively to enhance patient care.
Responsibilities
? Act as a liaison to ensure members are connected with appropriate care channels (i.e. pharmacy, insurance, billing, referral) and have the information they need to successfully navigate their care journeys.

? Conduct outreach and follow-up with patients in between episodes of care to promote a seamless healthcare experience.

? Ensure that members attend their appointments and assist with coordination for transportation and pharmacy needs.

? Collect all relevant information and relay it to the Care Coordination team.

? Participate in community-based events to represent CareFirst and promote health and wellness.
Requirements
? High School Diploma or GED
,
? 3 years of experience in community relations, outreach, customer service, or health education
,
? Strong written and verbal communication skills
,
? Problem-solving skills to tackle various tasks and challenges
,
? Ability to work effectively in a fast-paced environment with changing priorities
,
? Proficient in typing and working within various web-based platforms
,
? Reliable personal transportation for member engagement
