Responsibilities
? Provide monthly calls and support to patients enrolled in the CCM program.

Requirements ? Active LPN license (compact) , ? Experience in geriatrics or primary care , ? Ability to sit and talk on the phone for long periods of time , ? System requirements: Windows i5 (or newer) processor, Windows Pro (not Home)
