Statement of Purpose: The Remote Medical Liaison is responsible for medical liaison duties delineated below. This position works closely with other medical liaisons under the direction of the Clinical Manager. Promotes and practices MidlandCAN, LLC?s mission and values and follows its policies and procedures. 2. Ensures confidentiality is maintained by entire team regarding patient/client information in accordance with HIPAA, professional and departmental standards. 3. Obtains Chief complaint, and pertinent information regarding patient visit as well as required history of present illnesses, medication reconciliation, immunizations update, procedure codes for all telemed appointments. 4. Assess information obtained through telephone encounters, go over symptoms to determine proper management. (Self-care, in office visit or ER) 5. Prescribe medication request refills from pharmacy or patient on behalf of the provider. (all non controlled meds) 6. Assess positive STI?s lab results, create triage note for encounter, initiate treatment and if needed schedule for treatment 7. Prepares forms: Sleep study, Oneblood script, FMLA?s 8. Assists patient with medical supplies request, assigning DME company, preparing records of medical necessity. Navigate patient referred to Physical therapy 9. Perform SDOH assessment, navigate patient, create note and add procedure codes. 10. Documents all patient information and results accurately in the patient?s chart in a timely manner. 11. Prepares and distributes reports within the clinic and to the health care team as necessary. 12. Prepares records/documents in response to requests from other health care providers. 13. Requests diagnostic test results from outside laboratories and/or other health care providers. 14. Provides assistance with medical records. 15. Serves and protects the Organization by adhering to professional standards, institutional policies and procedures, federal, state, and local requirements. 16. Participates and strives to achieve assigned Annual Performance Objectives as established by Midland leadership. 17. Continuously demonstrates clinical competency with medical assistant skills and safety procedures and utilizes universal precautions where appropriate. 18. Adheres to ethical and legal standards of practice. 19. Performs all other assigned duties. Secondary Tasks: 1. Assess positive STI laboratory results and Monitors STI log. 2. Assists with the patient portal Healthvana messages. 3. Monitors Healthvana?s positive STI results. 4. Assists with medical liaison phone messages and coverage. 5. Performs radiology follow up calls and emails. 6. Prescribes all eRxs by end of day (all non-controlled), including ARV bridges. 7. Triages Novohealth patients when required. Value Base Care duties: 1. Perform document review notes for screening documents (ex; colon cancer screening, breast cancer screening, etc?) and adding procedure codes. 2. Stellar Health: Close quality measures gaps in Stellar Health. (Clinical alerts) 3. Assess patient assessment forms: IOA and MCAIP forms Administrative Duties: 1. Performs administrative duties. 2. Works with general office / clinic equipment (copier, fax, telephone, scanners, etc.) 3. Collaborates with Partner agencies as needed. 4. Performs billing and coding of services. 5. Obtain Electronic Medical Records (EMR) reports from outside providers and upload to patient chart. Education/Professional: 1. Highschool diploma or equivalent (GED) 2. Medical Assistant Certification issued by an accredited institution approved by the local state, or a minimum of 3 years? experience working in a health care setting. 3. Basic Cardiac Life Support (CPR) Certification 4. 1-2 years' experience of customer service 5. Electronic Medical Records training 6. Certified Medical Assistant (CMA) preferred Competencies: 1. Midland Required Trainings: General Orientation Violence in the Workplace Work Environment: HIPAA HIV/AIDS Sexual Harassment Health Stream Courses (as assigned) ? This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, audio visual, telephones, photocopiers, filing cabinets and fax machines. Physical Requirements: 1. Neat, professional appearance. 2. Sedentary- Exerts up to 10 lbs. of force occasionally and/or a negligible amount of force frequently or constantly in lift, carry, push, pull, or otherwise move objects. Involves sitting most of the time but may involve walking or standing for brief periods of time. 3. Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity. Position Type/Expected Hours of Work: ? This position may require additional time above normal operating hours and on occasion weekend work. Travel: Travel is primary during the business day, although some out-of-area and overnight travel may be expected. Must be able to operate a motor vehicle and have valid insurance and driver?s license.
