Key Responsibilities
? Conduct thorough assessments to evaluate patient needs.
? Develop tailored care plans that align with individual health goals.
? Coordinate effectively with physicians, hospitals, and various healthcare providers.
? Educate and guide patients and their families throughout their healthcare journey.
? Advocate for patients to ensure they receive optimal care.
? Maintain precise and updated patient records.
Required Skills
? Current Registered Nurse (RN) license.
? Minimum of 2 years? experience in case management, including managed care or home care.
? Strong communication and interpersonal abilities.
? Capacity to work independently as well as collaboratively within a team.
? Excellent organizational and time management skills.
? Bilingual skills (Spanish, Russian, or Chinese) are advantageous.
Qualifications
? Valid and active Registered Nurse (RN) license.
? At least 2 years of relevant experience in case management or a related field.
