BlueCross BlueShield of South Carolina is seeking an experienced Nurse Case Manager to join our team. We are looking for an individual who is passionate about helping others and has the knowledge and experience to excel in this role. The ideal candidate will possess a strong understanding of the healthcare industry, a commitment to patient care, and a desire to work with a team of healthcare professionals.The Nurse Case Manager will be responsible for providing care coordination services to members and their families. This position will involve identifying and assessing members' health issues, developing and implementing care plans, coordinating services, and providing education and support. The Nurse Case Manager must have excellent communication and organizational skills, as well as the ability to effectively collaborate with medical providers, patients, and families.Qualifications:? Bachelor's degree in Nursing or related field? Current, unrestricted nursing license in the state of South Carolina? Minimum of two years of experience in a healthcare setting, preferably in case management? Knowledge of healthcare systems, policies, and procedures? Excellent problem-solving and communication skills? Ability to manage multiple tasks and prioritize workload? Proficiency in utilizing computer systems and applications?
